Midwest Care Partners ACO is a collaboration between hundreds of regional healthcare providers, and our care team to provide additional support to our patients. Rising costs and fragmentation in how healthcare is delivered has made it difficult for patients or a loved one to navigate the healthcare system. By placing our skilled providers at the center of patient care, this partnership will make it easier for patients to manage their care journey through enhanced communication, better collaboration, and high-level care that should ultimately drive improved outcomes.
