What is a "Patient Centered Medical Home"?

Imagine that feeling of coming home—the safety and familiarity, the way your body just relaxes and your mind focuses on what’s really important to you. When you are home, you are surrounded by people who care about you—your teammates in life—and you face things together.

Now imagine a medical home—again, safe and familiar where you can relax knowing that the medical providers around you are on your team, working together and coordinating efforts to bring you the best possible health care. That’s what the Patient-Centered Medical Home (PCMH) model is all about.

Patient Centered Medical Home is a philosophy of delivering patient care so that it improves the health of both individual patients and the community as a whole, while also lowering healthcare costs. This is accomplished by focusing more attention on the patient and better coordination of care across every health provider that provides treatment.

There are four main components of a Patient Centered Medical Home model:

  • Comprehensive — Care is provided by a team and led by a personal physician focused on you—the whole you, including your physical and emotional well-being.
  • Accessible — Same day appointments, extended appointment hours and after-hour urgent care—plus 24/7 access to emergency rooms and your medical records.
  • Coordinated — Care is organized across all your providers (primary, specialty, lab, imaging, ER, urgent care, hospitalization and more) with enhanced use of technology to aid communication.
  • Committed to Quality & Safety — Proven-based best practices, tracking outcomes, and complying with regulations to ensure safety and the delivery of high-quality, cost-effective care.

The majority of primary care practices within HCA Midwest Health are designated patient centered medical homes by the National Committee for Quality Assurance. In a PCMH model, your primary care physician becomes your “medical home,” taking the lead on your care and coordinating with any other providers you need, inside or outside of that practice.

But it’s that model of care that impacts our patient’s health and satisfaction each and every day. That’s how we’re achieving patient satisfaction scores of 4.6 (out of 5), compared to the national average of 3.8.
