The heart care team includes medical professionals across many disciplines including internists, cardiologists, nurse practitioner, nurses, cardiac rehabilitation staff, dietitians, pharmacist, case managers, and staff from health information and emotional and spiritual care. From onset, to diagnosis, through the full heart failure treatment journey, we are dedicated to ensuring the best results for all patients.

The Research Medical Center heart failure management program includes:

  • Early stage heart failure management, where we follow the American College of Cardiology American Heart Association (ACC/AHA) guidelines for managing and treating heart failure.
  • A Heart Failure Coordinator that provides personalized outpatient care including evaluation of health status, medical optimization, health monitoring, education, and early referral for a device implant, if needed. This coordinator is a Nurse Practitioner that also coordinates all aspects of the program to ensure effective care and smooth transitions.
  • Complete management and follow-up care within seven days post discharge of heart failure patients.
  • A Heart Failure navigator who acts as an advocate for patients and their families, provides education, schedules follow-up visits and helps with questions or concerns.
