Our relationship with CareNow provides the opportunity to offer a seamless and positive experience for our patients who are treated at area locations. Following a patient visit, the care provider will send a visit summary to the patient's primary care physician and refer the patient back to the primary physician for ongoing, follow-up care—thus providing a continuity of care to enhance the patient's experience and improve outcomes. If a patient does not have a primary care physician, the CareNow staff will offer to help the patient locate one within the HCA Midwest Health family.
This partnership give our patients expanded options when the doctor is unable to see them for minor issues, especially after hours, weekends and holidays. CareNow will complement—never replace—the care provided by our primary care providers. Every patient benefits from an established relationship with a primary care provider. They are the point of contact for your care, coordinating, advocating and directing care to best meet your needs and ensure the best possible outcome.
